A 43 year old man complained of sudden blurring of vision, numbness of the right face, arm and leg, slurred speech, and gait instability 8 months prior to assessment. Initial examination revealed mild right hemiparesis. Bilateral carotid angiograms were normal. Cranial CT was normal. Eight months later our examination showed normal acuity and fields. The pupils were 5 mm and reacted poorly to light but well to near. Upward and downward saccades were paralyzed. Upward and downward smooth pursuit and the VOR were full. Attempted upward saccades elicited retraction pulses. Pain and temperature sensations were reduced over the entire right side of the body. The right arm showed moderate intention tremor and ataxia. He had slight pyramidal weakness of the right arm. Tendon reflexes and plantar responses were normal. Posterior circulation angiography showed a small vascular malformation in the caudal left thalamus. Therapeutic options will be considered. |
