Description A variety of disease processes involving the optic nerve result in Wallerian-like degeneration of optic nerve fibers. These processes include degenerations of the retinal ganglion cells and toxic, ischemic, and metabolic diseases affecting the axons. This survey will review the pathophysiology of axonal degeneration as applied to the optic nerve. The problem was first recognized by Augustus Waller 145 years ago. By demonstrating that nerve fibers in the distal stumps of severed nerves degenerate while those in the proximal stump to the injury survive, he provided one of the early cornerstones of cellular neurobiology'""? His observations laid the groundwork framework for the subsequent recognition that macromolecular synthesis occurs predominantly in the perikaryon (soma) of the neuron and for the discovery of axonal transport l 00 years later'"". The cellular responses that take place during Wallerian degeneration, including the elaboration of neurotrophins, are increasingly recognized to set the stage for the success or failure of subsequent regeneration, and manipulations of Wallerian degeneration are being investigated as potential means of altering the outcome of nerve regeneration.
